Highlights

Community Physician Network is seeking a Family Medicine physician to join our outpatient practice in Lapel, IN.

• Well established practice

• Shared call schedule with 24/7 RN triage

• Patient Centered Medical Home practice model with pharmacist, dietician, social work, etc.

• Electronic Medical Record (Epic)

• Ability to practice in a smaller community while having the support of a large network

Licensure & Certifications

• Board certified/board eligible in Family Medicine

• Licensed in Indiana or eligible for medical licensure in Indiana
