Intake Clinician
Monday - Friday - 1pm to 9:30pm
POSITION SUMMARY
The Intake Clinician is responsible for the facilitation of requests for services, resource referral, assessment of patient/family clinical needs, and interfaces with managed care organizations, external reviewers, and other payors.

Valle Vista Health System features individual units for children, adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs. Qualifications
QUALIFICATIONS
Education: Masters Degree in social work, counseling, mental health/behavioral sciences, or related field, or RN - preferably BSN.
Licensure: Licensed in field, preferred. Must have a valid driver’s license.
Experience: 1-2years of experience preferred in psychiatric health care. Working knowledge of DSM-V diagnostic criteria, mental status exam, and addiction and recovery field. Previous experience in mental health field, preferred.
POSITION SPECIFIC RESPONSBILITIES
Inquiry Calls
Documents relevant clinical, demographic and referral source information on the inquiry call form and enters call into MS4.
Provides description of Valle Vista Programming which could meet caller’s need.
Prescriptively recommends assessments to determine clinically appropriate level of care.
Answers inquiry calls quickly, professionally and communicates willingnessto help.
Level of Care Assessments and Admissions
Provides the initial evaluation, documenting relevant symptoms, functioning, supports and needs with appropriate detail and clarity.
Ensures patient safety through completing 15 minute observations and securing belongings as appropriate during the assessment and/or admission process.
Wand patients that prior to assessment and that are being admitted to unit.
Advocates for the most clinically appropriate level of care consistent with patient needs
Staff patient assessments with physician as appropriate and document recommendations along with preliminary DSM diagnostic impressions.
Enters intake and admission information into MS4 accurately and completely.
Completes admission documents timely and accurately and provides copies to applicable departments
Ensures timely, professional interactions with staff from other departments as patients are handed off to begin applicable programming.
Ensures timely completion for the assessment process with an average time of 90 minutes from the beginning of assessment.
Direct Admit Requests
Monitors Concord E-Fax to ensure timely response to direct admit requests within 15 minutes of receiving
Provides professional communication to all referral sources when responding to direct admit requests.
