Home Hospice RN - Days - Deaconess Linda White Hospice House

40 MILE RADIUS RULE - NO LOCAL CANDIDATES

Contract Length: 13 weeks

Start Date: ASAP

Guaranteed Hours Per Week: 36

Primary Location: Linda White Hospice House (Deaconess Hospice) - Evansville, Indiana

Shift Requirements: 8hr Days - 8:00 am - 5:00 pm

Float Requirements: Home Health Hospice, will need a reliable vehicle

On Call/Call Back Requirements: On Call Rotation For Call In's - On Call 5pm-8am (expected to call family back within 5 minutes and arrive to patients' home within 30-45 minutes) Call rotation is very limited and only based on call ins, or vacations all nurses are in the call rotation.

Weekend/Holiday Requirements: 1 Holiday. 1 weekend every 6 weeks (if needed)

Required Certifications to be Attached upon Submittal: IN or Compact RN License, AHA BLS

Minimum Experience: 1-2 Years Hospice Experience. 1 travel assignment required.

Charting Experience Required: Home Care Home Base (will train) – training modules can be completed prior to starting assignment in orientation

Required Skillset: Med Surg, Hospice, Critical Care

Unit Information:

About 30 miles or less a day. Some days may go over but they try to keep territories tight to reduce time in the car. They cover 5 counties: Vanderburgh, Warrick, Posey, Gibson, and part of Spencer.

Will keep the same patents for the entire 13 week contract. Case Load of 12-15 patients assigned in territory (some patients may pass away and may get new admits but will see the same patients every week). They have a stand up/stand down call everyday where they review schedules in case anything changes, but staff are not expected to come to the office every day. Expectation to be at the first visit or the office by 8:00am. Most nurses either do the stand-up call in their car outside their first visit or may stop by the office to grab some supplies. Occasionally PRN needs come up with patients throughout the day. If one of their patients calls in and has a need they will be expected to go, see them.

GSA hours are not guaranteed the week of Orientation. Traveler is guaranteed set hours per week, except during holidays where unit is closed
